Written Notification of Survivor Rights and Options

(Pursuant to 110 ILCS 155/15 - Preventing Sexual Violence in Higher Education Act)

Avid CNA School maintains a zero-tolerance policy toward sexual harassment, sexual assault, domestic or dating violence, stalking, or any form of sex-based misconduct. Although Avid CNA School is not a Title IX recipient, we are fully committed to maintaining a safe, respectful, and inclusive educational environment. We will assist and support any student or employee who reports sexual harassment or sexual violence.

1. Rights and Options Under Illinois Law (110 ILCS 155/15)

• Right to Report or Not Report: You may choose whether to report an incident to Avid CNA School and/or law enforcement.

• Assistance with Law Enforcement: Staff can assist you in contacting law enforcement if desired.

• Interim Protective Measures: You may request schedule changes, no-contact directives, or other protective measures.

• Access to Health, Mental-Health, and Advocacy Services: The school will help you access support and counseling resources.

• Summary of Procedures: You will receive a clear outline of the school's investigation and resolution process.

2. Institutional Commitments

Avid CNA School prohibits retaliation against anyone who reports or participates in good faith. Reports are handled in a trauma-informed and survivor-centered manner, and support is available even without a formal complaint.

4. Medical and Emergency Assistance
